Regressive Melanoma

 

Regressive melanomas have a very poor prognosis.  Histologically more classical melanoma cells may be absent, with remaining papillary dermal scarring and melanophages.  In the case above the pathologist read the initial slide as scar with melanophages and a diagnosis of melanoma was not provided.  I sent pictures of the case to him and he immediately agreed that this is a good example of a regressing or regressive melanoma.  Regression is a poor prognositic sign.  The photographs provide all of the key features of melanoma…Asymmetry, irregular Boarder, black to blue Color (with the white areas that show regression), and Diameter greater than 6 mm.  A right shoulder mass was present in this particular case with pulmonary metastasis on radiographs.
